Bausch & Lomb 90 Lensometer Model 21-65-90
Old and dirty, but seems like everything is here based on other people's photos, but since I don't really know anything about these I cannot attest to that or if it is functional in any way. The light and eyepiece is included and work as shown in the photos. I assume these are precise instruments, so I expect a lot of cleaning and calibration is needed.  And there is a good chance there is something wrong with it that is not obvious to the untrained eye that would just turn it into a paper weight.
